Privacy Policy

Evangel Hospital (“EH”) requests you to provide certain Personal data in order to access and/or use the EH Mobile App (the “App”).  Please read through the Notice before you furnish any Personal Data to EH.

 Notice

1.      EH is committed and will take reasonably practicable steps to protect the privacy, confidentiality and security of the Personal Data in accordance with:

a.      the requirements of the Personal Data (Privacy) Ordinance (“PDPO”);

b.      the guidance provided in the information leaflets and other publications issued by the Office of the Privacy Commissioner for Personal Data;

c.       any other relevant requirements under the laws of Hong Kong.

This Notice is in addition and without prejudice to any other notice provided to you at any other time you provide Personal data to EH.

2.      You need to register for an account and become an App member in order to use certain Services, in particular those which involve access to and management of Personal Data within the App.  To use the App or register an account via the App, you may be required to provide Personal Data to EH as indicated in the App from time to time which may include name, identity document information, date of birth and contact information.  You may also be required to provide additional Personal Data to enjoy designated discounts, e.g. employment status and church information, etc.  All these data are referred to as “Personal Data”. 

 

3.      EH developed the App to provide a portal for users to access and manage patient-related information as well as to perform certain tasks via mobile devices.  Your Personal Data may be used for the following purposes or any purposes directly relating to any of them:

a.      Purposes relating to your healthcare and/or generally for medical care or treatment purposes, including hospital fees;

b.      Education, research and/or statistical purposes;

c.       Operation and maintenance of EH information systems and the App;

d.      By virtue of PDPO, EH will not use your Personal Data for direct marketing purposes without your prior consent.  With your consent which is not subsequently opted out, EH may use your Personal Data for sending you promotional and direct marketing materials in relation to Services provided by EH or its affiliate through various communication channels (e.g. SMS, WhatsApp and electronic mail);

e.      Evaluation, planning and improvement of Services which EH provides;

f.        Communication with you;

g.      Accreditation, audit, service quality control and administrative purpose;

h.      Investigation and processing of complaints or disputes;

i.        Prevention or detection of crime;

j.        Compliance with or disclosure as permitted or required by any law, rule, regulation, code or guideline; and

k.       Any other purposes as may be agreed between you and EH, including purposes set out in any written communication or terms and conditions for the supply of specific Services.

 

4.      Please make sure that the Personal Data that you provided is accurate and complete.  If you fail to provide EH with the information required or if the information is inaccurate or incomplete, the processing of your account registration or log on process will be affected.  EH reserves the right to take appropriate action against a person who provide inaccurate information that may lead to potential loss of EH. 

 

5.      EH requests permission to collect, use and/or access certain features on your mobile device and personal information for some Services, including:

a.      the camera on your mobile device to take and upload photos of the requested information, if applicable, and conduct video conference;

b.      the camera or photo album on your mobile device for the account profile and other photos and QR code scanning;

c.       the microphone of your mobile device for conferencing and voice typing;

d.      the audio speaker of your phone for voice navigation;

e.      the calendar on your mobile device to record your appointment(s) and/or training activities in your calendar;

f.        your health data in other mobile applications for data consolidation; and

g.      data about your location to facilitate making appointment.  Your location can be determined with varying degrees of accuracy by GPS, IP address and information about things near your mobile device, such as Wi-Fi access points and Bluetooth-enabled devices, etc. The types of location data we collect depend in part on your device and account settings.

We will not otherwise access or collect any data of your mobile device or stored therein without your permission. You may provide and/or withdraw your permissions to the collection, use and/or access of the aforesaid personal information by the App at any time by changing the settings on your mobile device but this may affect you to receive certain Service(s).

 

6.      With your consent, we may use the push notifications on your mobile device to send you information, e.g. appointment reminder and seasonal messages.  You can deactivate these notifications at any time by changing the notification settings on your mobile device but this may affect you to receive notification of certain Service(s).

 

7.      Please also note that your Personal Data may be made available to appropriate persons:

a.      in or outside EH (e.g. third-party service providers of EH) who require it for matters relating to your App account creation and the provision of the Services, feedback, your case, carrying out of disease prevention projects and/or surveys or the purposes you are invited to provide data; or

b.      in any relevant government departments/appropriate authorities when EH is required to provide it under the relevant legislation for use for the purposes of that legislation.

In addition to the above, EH will use, disclose or transfer the Personal Data you provide via the App where required by law.  EH will obtain your consent before using or disclosing your Personal Data for any other purposes.

 

8.      EH will not transfer your Personal Data outside Hong Kong without your prior consent.  However, if App users access their Personal Data via the App when travelling abroad, they shall be aware that this may cause the transfer of their Personal Data outside Hong Kong.

 

9.      We have implemented various physical, electronic and management measures to safeguard and secure the Personal Data you provide via the App.   However, there is an inherent risk of wrongful, illegal or unauthorized access by third parties to your account(s) and/or handheld/mobile device(s).  You hereby acknowledge and agree to accept the risk of such wrongful, illegal, unauthorized access and hereby agree that EH shall not be liable for any loss or damage howsoever arising from such wrongful, illegal or authorized access.

 

10.  You have the right to request access to and/or correct your Personal Data held by EH.  Please make your request to EH Data Controller by:
